Free Medical Office Supplies Purchase Order Template & Generator
An outpatient supply order is a catalog-number document. Exam gloves, medium can be four products at four prices, so each line carries the manufacturer, the supplier's catalog number, the pack and case count, and the unit cost the practice was quoted. Items with a code date, such as analyzer control solution, get a minimum-dating requirement in months. Patient names never belong on the order, only the department and the requester.

Use-case checklist
What to include in a medical office supplies purchase order
Two failures show up in outpatient supply, and only one of them is about money. The money one is substitution: a distributor filling nitrile glove, medium from whatever is in stock can swap thickness, cuff length, and price without a single field on the PO looking wrong, and the practice finds out when the invoice is 20% higher than the quote. The other is dating. Control solution and reagents arrive with whatever shelf life the warehouse has left, and a kit delivered with six weeks on it gets opened, used twice, and thrown away, which quietly turns a $61.00 item into a $61.00 loss every quarter. Writing a minimum-dating requirement and a no-substitution rule on the line is what makes receiving able to refuse a box at the door.
- practice, supplier, requester, and PO number
- catalog number, manufacturer, item, and pack size
- quantity, unit cost, lot, and expiration requirements
- delivery location and substitution controls
- approval, tax status, privacy boundary, and invoice instructions

How to make a medical office supplies purchase order in 6 steps
Open the purchase order generator
Start from this page so the purchase order opens with fields and sample lines already set up for medical practices and outpatient office buyers. No account is needed to create or preview.
Enter both parties
Add your business name, address and contact details, then the supplier name and address exactly as they should appear on the record.
Add the medical office supplies lines
Replace each sample line with the real work or goods. Make sure the document shows practice, supplier, requester, and PO number, catalog number, manufacturer, item, and pack size, quantity, unit cost, lot, and expiration requirements.
Check the figures
Totals calculate as you type. Confirm quantities, rates, any tax line and the final total against your own records before you send anything.
Set the reference and dates
Give the purchase order a unique number and the correct date so it can be traced later and matched to related documents.

Ridgeview Family Medicine issues PO MO-1179 to Calder Medical Distributors on May 4, 2026, ordered by practice manager D. Ferro against contract pricing sheet CP-2026-1. Delivery goes to the clinical storeroom at the Elm Street suite, not the front desk, and the control solution line carries a dating requirement.

The line most often written wrong is the glove: medium, box of 200 without the catalog number and mil thickness lets the distributor fill from stock at a different price. The tax figure above is illustrative only, and whether any of these items is exempt for your practice is a question for your own accountant rather than the order desk.
When this document is the right choice
Use this purchase order to authorize a purchase before a supplier fulfills it. A purchase order comes from the buyer and authorizes an order. The supplier later sends an invoice that references the PO number for matching and payment.
Frequently asked questions
Two lines came back on backorder. What should the purchase order have said?
Three things. Whether partial shipment is acceptable or the order ships complete, a cancel-by date after which the practice does not want the item at all, and whether a substitute is acceptable on that specific line and at what price. Without a cancel date, a backordered case of specimen containers turns up eleven weeks later against a PO nobody remembers, long after someone bought a pack retail to get through Friday. Ask for backorder status on the order acknowledgement rather than discovering it on the packing slip.
Should the purchase order capture lot numbers, or does that happen at receiving?
Lot numbers come off the cartons, so they are recorded at receiving rather than predicted on the order. What the PO can do is require them: add record lot and expiration at receipt against this PO line to the items where a recall or an expiry sweep would matter, and give receiving somewhere to write them. That turns the order into the start of a trail instead of a standalone piece of paper nobody looks at again.
Can I put the patient or procedure a supply is for on the purchase order?
Keep it off. A purchase order travels to a distributor, sits in their order system, and often gets forwarded by email, so identifying patient detail has no business on it. Use the department, the room, the requester's name, and a cost center instead, which gives you every allocation you need. If a supply genuinely has to be tied to a case, make that link inside your own clinical record. What your practice is obliged to protect, and how, is a question for your own compliance adviser.
The invoice came back above the contract price on two lines. What on the PO helps?
The contract or pricing-sheet reference and the unit cost written per line. Naming CP-2026-1 on the order and putting $92.40 beside the glove case turns a price dispute into a two-column comparison rather than a discussion. Ask for the credit against the specific PO line rather than as a lump adjustment, so next month's order can be checked against the same numbers without reopening the whole account history.
What should we agree about returns before we ever need one?
The return window, the restocking percentage, who pays freight back, and which categories cannot go back at all once a carton is opened. Get those from the quote or the contract sheet and reference it on the order the way CP-2026-1 is referenced here, so the terms are attached to the document rather than to whoever answered the phone. Then when four kits of control solution arrive at the wrong levels, the conversation is about which clause applies. Terms vary by distributor and by item category, so read the ones you are actually agreeing to.
